How to Register as a New Patient
Health Checks and what to do before you leave your old surgery
Please call at the surgery, bringing with you your medical card or NHS number, the address of your previous Doctor, proof of identity and your address. Also bring a list of your repeat medication.
Before you leave your old surgery, please make sure you have a sufficient supply of any medications to continue taking until after your registration with us is complete. (We recommend a minimum of 14 days supply).
You will be asked to fill in a registration form, a new patient questionnaire, complete basic health checks using our Healthpoint height weight blood pressure and BMI calculation. You can also book an appointment for a health check with the health care assistant.
Your medical records may take time to arrive from your previous Doctor (especially if moving from England/Scotland to Wales). A health check will gather information and help us understand your medical needs. If you have complex medical needs, please also ask your previous surgery for a 'patient care summary'.
